Patient Assessments
Patient assessments are designed to capture patient information for a particular condition (such as COPD Assessment) or service (such as an Emergency Room, Hospital or Physician Office Visit). The Centers for Disease Control and Prevention (CDC) developed a Health Risk Assessment (HRA) designed for the Medicare Annual Wellness Visits. The patient well-being assessment is designed to improve the patient's overall health well-being. It captures the patient's determinants, barriers, symptoms and well-being status.
Patient assessments will be available for annual physician visits (like Medicare's Annual Wellness Visits) or can be completed and/or revised by the patient (or care giver) at any time.
