Well-Being Assessment

The patient well-being assessment captures the patient's determinants, barriers, symptoms and well-being status. Patient assessments will be available for annual physician visits (like Medicare's Annual Wellness Visits) or can be completed and/or revised by the patient (or care giver) at any time. The Centers for Disease Control (CDC) developed a Health Risk Assessment (HRA) that is widely used as a patient assessment.
