Well-Being Assessment

The patient well-being assessment captures the patient's determinants, barriers, symptoms and well-being status. Patient assessments will be available for annual physician visits (like Medicare's Annual Wellness Visits) or can be complete or revised by the patient (or care giver) at any time. The Centers for Disease Control (CDC) developed a Health Risk Assessment (HRA) that is widely used as a patient assessment.
