The patient assessments are designed to capture patient information for a particular condition (such as COPD) or service. The services could include a Emergency Room or Hopsital visit or a physician office visit (such as the Medicare Annual Wellness Visit). well-being assessment captures the patient's determinants, barriers, symptoms and well-being status. Patient assessments will be available for annual physician visits (like Medicare's Annual Wellness Visits) or can be completed and/or revised by the patient (or care giver) at any time.
