Medication Management Factors include:

  • Medication Compliance
  • Medication List - maintaining an up-to-date medication list, optimizing the number and administration of the medicine (pill a day vs. 4 times per day)
  • Understanding Medication Purpose - for each medicine
  • Understanding Potential Side Effects - for each medicine
  • Ensure Physician & Pharmacist Awareness - ensure each prescribing physicians and pharmacists have the entire Medication List
  • Ensure Appropriate Dosage - based on age, weight, condition, etc.
  • History of previous Medication side effects
  • Ensure labels & proper identification - ensure medicine doesn't get mixed up and take the wrong one at the wrong time
  • Medication Interactions - ensure their are no drug to drug interactions
  • Understand Other Interactions - food, alcohol consumption, sun exposure, etc.
  • Understanding of Instructions - timing, route, etc.

Medication Management factors are sub-elements of the Patient Activation factors which are elements of the psychosocial factors in patient recovery.
